• $150 – Intake / Psychological Diagnostic Evaluation (90791)
• $100 – Psychotherapy (30 minutes) (90832)
• $150 – Psychotherapy (45 minutes) (90834)
• $200 – Psychotherapy (60 minutes) (90837)  
• $100 – Group Therapy Session (90853)
• $200 – Family Therapy Session (90846/90847)
• $250 – Intensive Outpatient (IOP) (S9480/H0015)

Medical Codes and Descriptions

• $350 – Intake / Psychological Diagnostic Evaluation and Med Management (90792)
• $150- Med Office Visit (Low Complexity) (99213)
• $200 – Med Office Visit (Medium Complexity) (99214)
• $250 – Med Office Visit (High Complexity) (99215)

Payment Expectations
If you choose the Self-Pay option, payment is due at the time of service. We may also ask you to sign a Self-Pay Contract, confirming your responsibility for the full cost of the treatment.
