Should you believe that your rights have been violated or wish to submit a formal complaint, please fill out the form below. Our team will get in touch with you promptly. Rest assured, Forge Health will not take any retaliatory action against you for filing a complaint.

If you believe your complaint has not been resolved, please contact our Compliance Manager, Alicia McDermott, at compliance@forgehealth.com. If you still feel your concerns are not addressed, you may also reach out to the regulatory body for your state listed below. MASSACHUSETTS

Forge Health is a licensed health care facility in Massachusetts and falls under the jurisdiction of the Department of Mental Health and Licensing at the Department of Mental Health. If you would like to file a complaint and are willing to provide your name and contact information, there is an online form available at – https://www.mass.gov/service-details/complaint-forms.

If you prefer to remain anonymous, you may still file a complaint by calling the Department of Mental Health Complaint Hotline at (800) 221-0053. The hotline is available Monday through Friday, 8:45am – 5:00pm.
