At Forge Health, we are committed to ensuring that all individuals, including clients, family members, providers, and others impacted by our services, receive the highest level of care and attention. We value your feedback and are dedicated to addressing concerns promptly, fairly, and professionally.

If you have concerns regarding your treatment or believe that your rights have been violated, we encourage you to contact us. We take all grievances seriously and will work diligently to resolve them in a timely manner. Please note that Forge Health maintains a zero-tolerance policy against retaliation or discrimination for filing complaints or grievances.

How to Submit a Grievance or Complaint

If you have feedback or wish to voice a complaint, we encourage you to first address your concern directly with the relevant facility or provider. Open communication can often lead to quicker resolutions and help us address your concerns effectively. Please reach out to the provider or facility leadership at your treatment location.

If you are unable to resolve the issue with the facility directly, you can proceed with formal grievance submission, as detailed below. We take all feedback seriously and will ensure your concerns are addressed through the appropriate channels.

If your grievance remains unresolved or if you have a serious concern (such as allegations of abuse or neglect), you may escalate the issue to the Compliance Manager, Alicia McDermott, at compliance@forgehealth.com. The Compliance Manager will review your case and work with relevant parties to resolve the issue.

If your concerns remain unresolved after the Service Recovery process, you may choose to contact the appropriate regulatory body for further assistance.

You can submit a grievance to The Joint Commission via The Joint Commission’s website: www.jointcommission.org. Scroll down to “Filing a Complaint.” The Joint Commission also has a complaint telephone number at (800) 994-6610 where you can speak to a Joint Commission representative.

MASSACHUSETTS

Forge Health is a licensed health care facility in Massachusetts and falls under the jurisdiction of the Department of Mental Health and Licensing at the Department of Mental Health. If you would like to file a complaint and are willing to provide your name and contact information, there is an online form available at – https://www.mass.gov/service-details/complaint-forms.
